Required Documents
Completed Project Start form – prior to receiving payroll for processing.
Digital copies only of the below documents. Please keep the originals for your records.
A completed W-4 for each of your employees
A completed state withholding form
A completed I-9 form – verified and signed by you, the employer.
A completed weekly work voucher for each talent. If a weekly work voucher cannot be provided, we will need an excel breakdown including the following information:
Social security numbers
Birthdates
Dates worked
Hours worked
Pay rate information (if the rate is flat, hourly, or based on an 8/10/12 hour day)
Any wardrobe fittings, wardrobe or mileage reimbursements, etc.
If any of your employees are incorporated, we require:
Corporations – please provide:
A W-9 with the name of their Corporation and a valid EIN
Articles of Incorporation
LLCs – please provide:
A W-9 with the name of their LLC and a valid EIN
Articles of Organization
LLC Indemnification Agreement, signed
Copies of tax forms 8832 or 1120
If talent payments are to be sent c/o their agency, we require:
A check authorization form, signed by the talent
If paying agency fees through our company, we require:
A list of the agencies on the job and which talent they represent
Contact emails for each agency
Current copies of each agency’s W-9
Percentage that the agencies will be paid
If paying minors, we require:
A work permit
If paying minors in the work states of California, New York, Louisiana, and New Mexico, we require:
Minor Trust/Coogan Account information (name of bank and account # required) Please note: A Minor Trust/Coogan account is required for work done in the states listed above. If the minor resides in a different state but worked in one of the above states, they are still required by law to have a Minor Trust/Coogan account.
